Action Home Health Care is a high quality home health care service organization comprised of an interdisciplinary team of dedicated medical professionals operating in a challenging and rewarding environment. We are committed to providing high quality, comprehensive, individualized and consistent care according to the physician’s Plan of Care.

Responding to growing market demands for Medicare homecare services, Action Home Health Care was established in 1995 as a corporation, licensed in the State of California, and soon after received Medicare/Medical certification as a home health agency in 1996. We have accreditation from the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).

At Action Home Health Care, we value our close relationships with hospitals, physicians, case managers, discharge planners and community agencies. Together with our staff, they have contributed to our distinct standards of quality and dedication that are important to patients as they make decisions about their healthcare needs.

The effectiveness of the familiar home setting during a patient’s continuing need for medical and rehabilitation care has been well documented as often contributing to a patient’s physical, emotional and psychosocial sense.

Action Home Health offers quality home care services 24 hours per day, 7 days per week. Office hours are Monday-Friday 9:00 a.m. to 5:30 p.m.
After hour emergencies are handled by our on-call Registered Nurses.

Professional care provided in the comfort of your own home

Action Home Health is dedicated to providing skilled, professional care to its’ patients.
As appropriate, agency professional staff visits the patient’s home to evaluate short-term and long-term health care needs. They work with the patient, their family, caregivers and the physician to develop a plan of care to meet each patient’s individual requirements. Referrals for additional services, equipment and supplies not covered directly by the agency may be referred to other providers.

Professional visits for skilled care are provided by a Registered Nurse (RN) or Licensed Vocational Nurse (LVN), Physical Therapists (PT), Occupational Therapists (OT) and/or Speech Language Pathologists (SLP), Medical Social Workers (MSW) make home visits to assess a patient’s and family’s needs and make referrals for community services. Our Certified Home Health Aides (CHHA) function as an extension of the skilled nursing and rehabilitation staff assisting patients with their personal care needs.

All services offered and arranged by Action Home Health Care are designed to help patients obtain their maximum functional level in the comfort of their homes. A Registered Nurse (RN) is available 24 hours a day, 7 days a week to respond to patient’s questions.

Action Home Health Care provides intermittent visits and extended (shift) care under Medicare, MediCal, Private Insurance and Self Pay Plans on an hourly or per visit basis.

Our Professionals are truly something special.

Registered Nurse (RN)

Makes the initial evaluation visit and regularly re-evaluates the patient’s nursing and health care needs. The RN is available to answer patient questions related to the care being provided and make periodic visits to assure the care is following the plan of care and that the plan of care remains appropriate to the patient’s medical needs.

The RN may also be evaluating the skills of the personnel providing the services including supervision of home health aides and other nurses, and assessing patient needs after hospitalization as applicable.

While the RN is the person primarily in charge of the care being provided by the Agency, personnel are available through the Action Home Health Care office to assist patients should they have questions or concerns.
A Registered Nurse provides clinical assessment, individualized instructions and establishes a nursing care pan in collaboration with the patient’s family and physician.

This plan of care states the specific services, treatments and medications related to the patient’s specialized needs.

Our nurses evaluate the effectiveness of the care plan through observation, of the patient’s condition, behavior, signs and symptoms of illness and reaction to treatment. Through communication with the patient, physician and health care team, the care plan is reviewed and modified as needed.

Licensed Vocational Nurse (LVN)

Provides those services requiring nursing skills within their scope of practice.  Services include monitoring the patient’s condition, notifying the agency and physician of changes in the patients condition, implementing the orders on the plan of care and plan of care updates, participating in the coordination of care with the Registered Nurse, physician, and/or appropriate others, and teaching the patient and family/caregivers about meeting their healthcare needs.

The LVN functions under the supervision of the Registered Nurse according to the physician’s Plan of Care and as authorized by the payer.  At times, services provided by an LVN may be provided by an RN.

Certified Home Health Aide (CHHA)

Provides assistance with personal care services such as bathing, personal hygiene, activities of daily living and simple dressing changes.  They work under the direction and supervision of the Registered Nurse or Therapist (if only Therapy is involved in the provision of care.)  The CHHA is generally only provided when there is no other caregiver in the home, the patient is unable to complete his or her own care and/or the caregiver needs respite.

Generally , the Home Health Aide is not covered if the patient is also getting IHSS services.  For Medicare, the CHHA services are not covered unless there is an ongoing medical need for the RN, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.

Medical Social Worker (SW)

Provides services that include help with social, financial and/or emotional concerns related to the patients illness or interfering with following the plan of care.  This may include counseling or referral to community resources.  Our Social Workers are primarily educated at the Master of Social Work (MSW) and the Licensed Clinical Social Worker (LCSW) level.

Medical Social Worker’s provide skilled assessment of both social and emotional factors related to the patient’s illness, need for care as well as response to treatment.

In collaboration with the health care team, the MSW evaluates the home and family environment in order to provide guidance and problem solving interventions.  MSW’s are available to coordinate outside community resources when applicable.

 Durable Medical Equipment (DME) and Pharmaceuticals- The nursing personnel and therapists may assist in coordination of delivery of physician ordered DME and, in some cases, pharmaceuticals
